Care Plan Resource for Chronic Disease Management

Hello,

Any advice on using the “care plan” resource to create treatment plans for chronic disease patients? I don’t see specific examples for chronic disease treatment plan on the FHIR website - the closest I was able to find is the one from Mayo (http://hl7.org/fhir/careplan-example-integrated.html). I would appreciate if someone can share care plan examples for Diabetes (including specifying a schedule for screenings, tracking vitals etc.) or any other chronic condition.
