I am trying to convert CCD to FHIR resources but not able to find any documents or help.
I guess people are already doing this, So If anyone has any documents or links or any sort of help then please share.

The recommendations are documented on the Mappings tab of every resource. Scroll down to the sections on CDA and RIM. You will find some gaps though.

I am referring to those mappings only. But as you said there are some gaps in those mappings as every field is not mapped. But I think for the time being that should work for my use case.
