How would we track the timeline that is associated with a patient and their PCP? Let’s say a patient is created on 1/1/2021 with a link to PCP ABC. On 4/1/2021, we received a change to use PCP 456.
Relationships between Patient and their providers can be represented using CareTeam - which includes information about the period of time when a particular Practitioner held a particular role.
