the discription of the Encounter.reasonReference and Encounter.diagnosis.condition in the specification may mixed up with Encounter.reasonCode
Agree, this does look like a problem. Can you use the ‘propose a change’ link at the bottom of the spec to request a fix? (There’s a one-time registration, but it’s free.)
