Resource access from several organizations: Introduction & best practices

Sorry for the beginner’s question, but I am really lost. We want to create an EHR from zero, where several clinicians from different organizations are involved in the treatment sequence of the patient. I know from several posts that access policy is not deliberately covered within FHIR. However, can somebody refer me where this problem is tackled?

Where do I look for concepts that solve this? Are there best practices around? Which kind of consultant do I need?

Have you had a look at the FHIR security section of the wiki. Also have a look at the Authorization/Access Control link in case it might be useful to some extent.
